Questionnaire Instructions

Please complete one questionnaire for each vendor. Your feedback will be used to assess vendor performance and inform contract negotiations for the upcoming year. Rate each item on a scale of 1-5 (1 = Poor, 5 = Excellent) and provide specific comments where possible.
